Rate and Payment Policy
My standard rate is $175 for 50-minute individual session and $250 for 75-minute couple and family session. Initial individual session fee is $250. Payment is due at the time of services. Cash or check is accepted. Sliding scale on limited basis is available. There is a 24-hour cancellation policy. Clients who do not give 24-hour notice of cancellation of scheduled appointment will be charged half of the session fee.
Insurance
I am a provider for the following insurance groups: Aetna, Anthem Blue Cross, Blue Shield of California, Beacon Health Options, Cigna, Magellan, HMC Healthworks, Sutter EAP, United Behavioral Health, and Wellspring EAP. I can bill Aetna, Blue Cross, Blue Shield, Beacon Health Options, Cigna, Magellan, HMC Healthworks, Sutter EAP, United Behavioral Health, and Wellspring EAP directly. I can provide Superbill or receipts for reimbursement if you have other insurance coverage. Please check with your health insurance company to best understand your coverage.
- Is there coverage for mental health services? If so, are there any limitations on the coverage?
- Is there coverage for out of network psychotherapist?
- Are there any limitations on the frequency of session (weekly or yearly)?
- What is the percent or dollar amount the insurance cover for each session or a year?
- Does your insurance require referral from the primary care doctor?
Also, some employers offer Employee Assistance Program, EAP that may cover the cost of counseling services for you and your family. Please check with your human resources or personnel office for information.
